Supportive
Care
Patients with life-threatening,
chronic, or debilitating illnesses
often have complex needs that
require the coordination of
medical, social and spiritual
services. Through St. Mary's
Hospital’s Supportive
Care program, the focus of
care for these patients can
be on quality of life though
relief of suffering, control
of symptoms, and when possible,
restoration of function.
The Supportive Care Team works
closely with patients, their
loved ones and primary care
physicians to ensure continuity
of care, address symptom control
issues including pain, and
provide support for their
emotional and spiritual needs
as well as their physical
needs. The Team can provide
assistance with decision-making,
discharge planning and counseling
for families and also assists
healthcare professionals in
preparing patients and their
families or friends for end-of-life
issues.
